The Lead Social Worker (SW) works within the context of the Central Florida Behavioral Health Hub (CFL-BHH), from a team approach, to coordinate clinical activities pertaining to the successful operation of the hub. The Lead SW serves as a team Lead in coordinating services with the interdisciplinary team and community agencies to ensure appropriate patient care across the healthcare continuum. The Lead SW provides clinical supervision to social workers in the CFL-BHH. The Lead SW provides expert social work evaluation and treatment services to patients and families with complex psychosocial needs independently and demonstrates commitment to a patient and family-centered model of care. The Lead Social Worker will perform the essential fun
