Hospice Social Worker – Full-Time, Tuesday thru Saturday, 40 hrs.

As a full-time Hospice Social Worker, you will provide medical social services to hospice patients, families, caregivers as indicated by the plan of care developed by the interdisciplinary team. Your caring visits will support and empower them to live the best quality of life possible. We’re looking for a social worker that wants to use their passion and expertise as part of a team to provide the best care possible for hospice patients.
Responsibilities include initial and ongoing assessment of patients, developing, evaluating, and revising the plan of care, timely documentation (written and electronic) and communication of assessment, planning and interventions. This position functions as an integral part of the interdisciplinary team.
Duties
Perform initial and ongoing assessment of patients, developing, evaluating, and revising the plan of care as needed to maximize your patient’s quality of life
Timely documentation and communication of assessment, planning and interventions
Function as an integral part of the interdisciplinary team
Utilize a variety of techniques to identify and assess the psychological, emotional, and social factors affecting the patient and family and communicate these needs to the interdisciplinary team
Provide education and information to patients and caregivers that facilitates self-care, wellness, dignity, and quality of life
Facilitate collaboration/communication with community resources
Act as liaison to promote discussion between the patient, family, and health care team, in order to facilitate understanding of the patient’s medical condition and treatment options, and to establish clear, patient-centered goals of care and care plans
Participate as a member of the larger Hospice Care Team, communicating clearly and effectively with other team members to provide quality patient and family care, meet individual patient and family members care needs, and achieve appropriate hospice care plans.
Requirements
Master’s degree in Social Work from a school of social work approved by the Council of Social Work
Demonstrated experience working with multigenerational family systems in a clinical capacity
Demonstrated experience working with life-threatening conditions, chronic disease, hospice and/or end-of-life care, and related situations
Current Oregon driver’s license, dependable transportation, and proof of current automobile insurance
