								<description>Stockton, California,  Summary The incumbent - an entry-level social worker - functions as one of the HUD-VASH Social Workers for the Medical Center&#39;s Health Care for Homeless Veterans Program (HCHV) - HUD-VASH Program. The HUDVASH program is a collaborative effort - supported through HUD Section 8 rental assistance vouchers and VA&#39;s provision of intensive case management services. The primary goal of HUD-VASH is to move veterans and their families out of chronic homelessness. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy Education: Have a master&#39;s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE) Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited A doctoral degree in social work may not be substituted for the master&#39;s degree in social work Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master&#39;s degree level Exception: VHA may waive the licensure or certification requirement for persons who are otherwise qualified - They work with the patient?s medical provider and, when appropriate, other mental health providers for an assigned caseload. Developing patient care goals and treatment plans as agreed upon by the patient?s primary care team and psychiatric consultant is a responsibility of the BHS. The BHS is also available to primary care teams as a consultant for those patients with complex psychosocial needs. Job duties:   Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care alongside the patient?s medical provider and, when appropriate, other mental health providers.   Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.   Provide patient education about common mental health and substance abuse disorders and the available treatment options.   Systematically track treatment response and monitor patients (in person, by video or by telephone) for changes in clinical symptoms and treatment side effects or complications.   Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. The Social Worker Program Coordinator is independently licensed at the Advance Practice level to the Health Care for Homeless Veterans (HCHV) Contracted Residential Services (CRS). The HCHV CRS Liaison is responsible for the monitoring and coordination of services provided by community-based programs funded under the VA emergency housing contracts. Duties Provide supportive services and case management to Veterans residing in Contract Emergency Residential Housing facilities Utilizes various treatment modalities of individual and group work - casework - community organization and community outreach activities to alleviate or remove problems that interfere with the Veteran&#39;s ability to achieve independence Monitors the clinical care and asses the compliance of the programs receiving payments - as described in the recipient&#39;s contract When necessary - the incumbent intervenes to facilitate compliance or corrective actions and - when appropriate - develops - facilitates and monitors plans of correction Provides oversight of the Contract Residential Services as defined by VHA policy Serves as the Contracting Officer Representative (COR) for the Contract Emergency Residential Housing Program - upon completion of required training and certification Coordinates care and acts as an advocate on behalf of Veterans to ensure comprehensive service delivery and provides linkages for Veterans to VHA health care services - Veterans Benefits Administration (VBA) and community agencies Provide a wide range of services - including assessments - referrals - short-term case management - advocacy - and service coordination Provide a full range of social work services within commonly accepted standards of social work practice which includes case work - individual - family and group counseling - discharge planning - case management and other services as appropriate Provide consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on healthcare - adherence to treatment - and housing plans Participate in conducting team conferences to promote professional development - training - and coordination of service delivery TRAVEL REQUIRED: Occasional by car to travel to Veterans Contract Residential Services Programs and or in the community to assist Veterans with housing as needed and also to conduct site visit of contract agencies Work Schedule: Monday-Friday 8:00 am to 4:30 pm Compressed/Flexible: Available Telework: Authorized for ad-hoc only after first 90 days of employment Virtual: This is not a virtual position Position Title/Functional Statement #:Social Worker (Program Coordinator)/PD000000 Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Financial Disclosure Report: Not required</description>
