Overview

• Conduct crisis assessments and provide therapeutic interventions to support patients in acute distress.
• Facilitate patient participation in treatment planning, education, and discharge processes.
• Collaborate with healthcare team members to ensure safe, timely, and effective care delivery.
• Assess psychosocial risk factors based on prior functioning, support systems, and reactions to illness or trauma.
• Counsel patients and families on emotional, social, and financial impacts of mental health or disability.
• Access and coordinate community resources, including protective services, foster care, guardianship, and mental health placements.
• Serve as the primary point of contact for cases involving abuse, neglect, domestic violence, and other protective issues.
• Provide intervention in guardianship, foster care, and adoption cases.
• Maintain documentation and manage all functions related to utilization review and discharge planning.

• Education: Master's Degree in Social Work, Counseling, Psychology, or a related field required.
• Licensure: Current licensure or license eligibility in Indiana as an HSPP, LMFT, LCSW, or LMHC required.
  • Clinical licensure preferred.
• Certifications: CPI (Non-Violent Crisis Intervention) certification may be required.
• Experience: Relevant healthcare, mental health, or community-based experience preferred.
• Knowledge & Skills:
  • Knowledge of the impact of disease processes and human behavior for assessments, crisis intervention, and counseling.
  • Understanding of state and county legal mandates related to child and adult protective services.
  • Principles of growth and development across the lifespan.
  • Strong crisis de-escalation and intervention skills.
  • Ability to develop and maintain strong community, law enforcement, and agency relationships.
  • Proficiency in Microsoft Excel and Word.
