DescriptionSummary:This position is to coordinate individualized care plans for patients who have had frequent visits to the Emergency Department for treatment of chronic pain and for patients who have had frequent readmissions to the hospital which will result in a consistent approach in developing the plan for treating patients with chronic pain. Responsibilities:Develop patient focused care plans defined by 6 ED visits in a year or 3 visits in a month or 3 hospital admissions in a year for chronic painCoordinate referrals to the Facilites Care CommitteeBased on the Facilites Care Committee send the individualized care plan via certified mail to the pain, the patient's physician and if the pat
