Job DescriptionCoordinate Behavioral Health care within the PCP or Specialty Care Practices, provide proactive follow up, monitor progress of person receiving care, and alert providers when additional or adjustments in treatments are indicated. Support appropriate medication management and facilitate communication with psychiatric prescribers.
