The incumbent services as the behavioral health co-located provider within the Patient Aligned Care Team (PACT). The incumbent provides "on-time, on-target" information to Primary Care Providers (PCP's), and collaboratively makes appropriate care decisions. He/she functions as a treatment advocate and guide, consultant, counselor and educator while providing integrated health care to outpatients in Primary Care.
