Mental Health Therapist
The Mental Health Therapist provides clinical mental health services to assigned patients in the Adult Partial Hospitalization Program (Adult PHP), including Group, Individual, and Family Therapy, Admission and Discharge Assessments, Case Management, and Discharge Planning. The Mental Health Therapist also acts as a liaison between the Adult PHP and community providers and between the patient, family, and the Adult PHP team and assists other members of the treatment team in coordinating and implementing the patients' individualized treatment plansRESPONSIBILITIES Completes psychosocial on assigned patients and assists treatment team in developing each patient's individualized treatment plan.
