The Outreach Social Worker is assigned to the Veterans Affairs Medical Center and provides services to patients/significant others treated in the Homeless Veteran Program. The social worker provides outreach, assessments, referrals, and case management services to homeless Veterans identified in the community. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
