Schedule: Full time days; Mon/Wed/Friday 8a-4:30p, Tues/Thurs 12p-8:30p; No weekends, No holidays

Location:  UChicago Medicine AdventHealth New Day Center (Substance Abuse Rehab) - 740 Pasquinelli Drive, Ste. 104, Westmont, IL

The role you’ll contribute:

Acts as liaison between patient, family, and care team coordinating plan of care.

The value you’ll bring to the team:

·       Creates plan for care across the continuum, integrating patient/family preferences and values.

·       Monitors patient and family care.

·       Advocates for resources and removal of barriers.

·       Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

·       Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

The expertise and experiences you’ll need to succeed:

One or more of the following Required: 

·       Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

·       Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

·       Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

·       Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

BLS Provider credentialed from the American Heart Association (AHA) obtained within 3 Months (90 days) of hire date or job transfer date.

CADC - Certified Alcohol Drug Counselor certification highly Preferred.
