Part-Time Saturday and Sunday 7a-7p or 8a-8p

(Two 10 hour days eligible for benefits)

JOB SUMMARY:

Provide comprehensive biopsychosocial assessments and consultation to patients and families as a member of the health care team. Assist families in coping with patient's illness, diagnosis, treatment, and/or life situation. Provide referrals to link patients and families with available resources in the community to help then maintain optimum wellness.

ESSENTIAL FUNCTIONS OF THE JOB:

1. Identifies high-risk individuals. Performs comprehensive assessment to determine needs as well as other psychosocial interventions that may be necessary.
2. Establishes a plan that addresses patients' needs, in coordination with the patient, family or caregiver. Reevaluates the plan and revises as needed.
3. Conduct social determinants of health (SDoH) assessments and develops individual plans to address findings.
4. Assists with identification, management and treatment of victims of abuse, neglect, domestic violence, etc.
5. Advocates for community members rights to culturally appropriate healthcare.
6. Oversees completion of Advanced Directives for patients & community members.
7. Collaborates with interdisciplinary team as needed to insure patients receive the care in the most appropriate setting.
8. Maintains a working knowledge of relevant medical/legal issues that affect patient care, e.g., advance directives, child and elder abuse.
9. Provides education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.
10. Maintains working knowledge of and liaison with community agencies and resources.
11. Assists in maintaining continuity of care and preparing patient and family in the transition of the patient to a more appropriate level of care when medically indicated.
12. Documents assessments, interventions, changes in patient's status and plan.
13. Ensures compliance with all regulatory requirements.
14. Performs related duties as assigned.
