Makes referrals to approach social service agencies and arrange for placement as directed. Consults with MSW SW as needed for clinically complex patients or difficult family dynamics.What You Bring/Job Requirements:​In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:* Social Work, Professional Counseling, Marriage, Family and Child Counseling* 1-3 years of experience in Social Work with emphasis on discharge planning, referral to community resources and/or case management.* Hospital based experience preferred. 2 years social case management desired hospital setting preferred.* Licensure *(SSW required)* in the state of employementPhysical Requirements - Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantlyYour Connected Community:​When people and places align, incredible things happen.
