Overview

This position provides support to patients through education and coordination of resources, support programs, and services to assist with health related needs. The Case Management Coordinator guides patients and their families through the healthcare continuum by identifying barriers to care and assisting with access to needed resources to decrease barriers. This position performs follow-up tasks identified during the clinical/hospital assessment for management of patients across the healthcare continuum or when the patient is in the continuum and needs additional resource support.

Position Available:

Full Time

40 Hours per week

8 Hour Day Shifts

Monday - Friday

Responsibilities

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Foundation Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.

Qualifications

High school diploma/GED or equivalent working knowledge.

The position requires a proficiency level typically achieved with 3 years of experience in healthcare as a Nursing

Asst, Medical Asst, Health Unit Coordinator, Patient Care Tech, etc.

Requires an understanding of medical terminology. Must demonstrate effective communication skills, human relations skills, strong organizational and time management skills and flexibility in responding to multiple demands

PREFERRED QUALIFICATIONS

Bilingual, preferred in some assignments.
