The Healthcare for Homeless Veterans Program Coordinator at the GS-12 grade serves as the program coordinator and is administratively responsible for the Healthcare for Homeless Veteran (HCHV) Program. The HCHV Coordinator coordinates outreach to homeless Veterans across the state of Montana including but not limited to scheduling, coordinating and participating in community Stand Down events statewide. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Preferred Experience: Experience working with the Homeless population, Veterans, Mental Health and Substance Disorder and Experience with mentoring staff and/or providing program oversight. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. ["As a Healthcare for the Homeless Veteran Program Coordinator the incumbent will have the duties listed below: Participates in the evaluation and selection of potential CES facilities as well as the development of the contract. Provides supervisory oversight of the CES facility contracts and provides administrative, technical and clinical supervision necessary for accomplishing the requirements of the contract. Establishes guidelines and monitors contractor progress on performance expectations. Resolves informal complaints and grievances from Veterans and/or contractors. Provides advice and counsel to contractors related to work and administrative matters. Provides technical assistance to CES facilities which involves monitoring activities to ensure compliance of facility programming with the specifications outlined in the contract. Ensures standards are maintained to promote the health, safety and welfare of homeless Veterans. When necessary, intervenes to facilitate compliance or corrective actions and develops, facilitates, and monitors corrective action plans. Initiate and enforce suspension or withholding of payment for services when indicated until the CES facility has adequately corrected any deficiencies. Use sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Perform program analysis, development, review, research and design. Provides a wide variety of case management activities including screening of referrals, verifying eligibility for VA services, coordinating care and participating in admissions, treatment planning and discharges. Responsible for ensuring timely responses to homeless consults in CPRS and the National Call Center for Homeless Veterans (NCCHV) according to required standards. Responsible for the assessment, identification, facilitation, referral and linkage to appropriate medical and psychiatric services within the VA and the community. Ensures that all interactions with the veteran are documented according to facility policies and procedures in the medical record (CPRS) and the Homeless Operations Management and Evaluation System (HOMES). Completes a comprehensive psychosocial evaluation of all Veterans at entry into HCHV services. Administers informational and analytical systems to evaluate and enhance the quality of services provided to veterans. Manages and controls the use of data regarding day to day functioning and program evaluation. Incumbent develops and implements data tracking systems to track services provided to veterans, patient demographics, and patient satisfaction. Develops, implements and evaluates services using clinical guidelines, protocols, and directives to establish appropriate utilization of services. Establishes and maintains monthly billing protocols according to contract specifications and fiscal management protocols. Reviews and evaluates facility invoices, vouchers and census tracking to ensure accurate billing for services. Determines approval of continued payments based on the CES facility meeting the standards and requirements as outlined in the contract. Provides subject matter consultation to colleagues, students and community providers on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise. Models effective counseling practice skills and mentors staff and students in the specialty area of practice. Participates in local and regional Homeless Coalitions, Continuums of Care, Coordinated Entry Systems and represents Homeless programs and the VAMC.
