This is high volume, customer service environment. You’ll need to be efficient, productive, and thorough dealing with our members over the phone. Solid computer and software navigation skills are critical. You should also be patient-focused and adaptable to changes.
This position will require active and unrestricted Nursing licensure in multiple US States. Selected candidate must be willing and able to obtain and maintain multiple state licensure. Application fees and filing costs will be paid for by UHG.
This is a hybrid position. The typical schedule includes working on site in our office located at 11030 Optum Circle, Eden Prairie, MN 55344 three days/week and at home two days/week.
The schedule for this position is Monday - Friday during standard business hours and includes working two nights per week until 8PM CST.
Primary Responsibilities:
- Making outbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self-management
- Interacting with Medical Directors on challenging cases
- Coordinating care for members
- Making referrals to outside sources
- Coordinating services as needed (i.e., home health, DME, etc.)
- Educating members on disease processes
- Encouraging members to make healthy lifestyle changes
- Accurately and timely documentation of member progress
- Making 'welcome home' calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:
- Active, unrestricted RN license in the State of Minnesota
- Willingness and ability to obtain additional RN licenses as necessary
- 3+ years of RN experience in a hospital setting, acute care, direct care, or as a telephonic case manager for an insurance company
- Demonstrated computer proficiency including navigating a Windows environment; using MS Outlook; creating, editing, saving, and sending documents in MS Word; talking and typing at the same time; and using multiple applications simultaneously
- Willingness and ability to work on site in our Eden Prairie office an average of three days per week
- Access to secure, high-speed internet and a dedicated, distraction-free workspace at home
Preferred Qualifications:
- Bachelor's degree
- Certified Case Manager (CCM)
- Managed care experience
- Case management experience
- Call center experience
- Experience with/exposure to discharge planning
- Experience with utilization review, concurrent review and/or risk management
